Instead of going through insurance, which often dictates care through pressure to see a high volume of patients, concierge medicine involves paying a monthly or annual fee that removes insurance from the equation, so Travis can work solely for you—ensuring that every decision is grounded in what truly supports your optimal health and well-being.
Travis is deeply committed to the care of older adults, taking the time to get to know each patient and caring for them as he would a member of his own family. Understanding that aging comes with challenges to mobility and increased health needs, Elder Pathways concierge includes unlimited home visits Monday through Friday, with 24/7 phone calls, emails, and text messages—so you receive comprehensive, personalized primary care whenever and however you need it.
To preserve this level of attentiveness, enrollment is intentionally limited. This ensures that patients have reliable access to same-day appointments, as well as direct communication via phone, email, or video conferencing. We believe your time with your healthcare provider should feel unrushed, connected, and responsive—because your concerns deserve timely attention and care that’s truly personal.
The term "elder" refers to an older adult but it also signifies more than just chronological age—it conveys a sense of honor, dignity, and value for a person’s life experience and contributions.

Elder Pathways is a concierge medical practice founded by Travis Neill, a board-certified Physician Assistant with specialized training in geriatrics and over 15 years of experience caring for older adults. Based on a deep respect for the power of family and the importance of meaningful connection, Elder Pathways delivers highly personalized care through a small, close-knit panel of patients—treated like extended family.
Travis draws on his extensive clinical experience and a trusted network of specialists and geriatricians to provide patient-centered guidance through the final stages of life. His approach is deeply human, honoring each person’s unique values, goals, and fears. When asked why he chose geriatrics, Travis shares:

Membership with Elder Pathways Includes:
Unlimited home visits Monday through Friday from 8am-5pm with messaging access 24hrs a day 7 days a week. Same day visits or within 1-2 days following a request for a visit
Cross coverage from an experienced Physician or PA when Travis is on vacation or unavailable
Continuity of Care - will follow you if you move within a 15 mile radius of your current location
Coordination of care with specialists, therapists, and other necessary services. Referrals and labs with access to a comprehensive network of specialists for all healthcare needs
Initial review of all medications with a focus on risk vs benefit analysis and ongoing medication management
Wellness programs for sleep, diet, and exercise
Procedures such as joint injections, ear cleaning, wound care, and cryo freezing skin lesions with no extra fees.
Access to a patient portal for all medical records, billing, and communication to ensure seamless comprehensive care
Caring for older adults involves understanding the aging process while coordinating care with specialists and taking on a more central role in diagnosing and treating acute illness and managing chronic diseases.

Our current reimbursement in healthcare is based on a fee paid by insurance companies for every visit. The reimbursement rate has declined and insurance companies are often directing care decisions. Travis believes that quality primary care for older adults requires a fast response to needs and more time to discuss goals, coordinate treatments, and focus on prevention. Concierge medicine provides direct access to your Primary Care Provider and timely visits in your home. This is particularly important for seniors when mobility becomes more challenging. This type of care translates to less unnecessary hospitalizations and interventions while ensuring your care aligns with personal values and priorities.

Travis brings 15 years of experience in geriatrics and has been mentored by a team of esteemed physicians with a collective expertise spanning over a century. He maintains close collaboration with specialists and his mentor and adviser Dr. Greg Gahm—a Harvard-trained, award-winning geriatrician. Travis has also served for several years as Assistant Medical Director for multiple skilled nursing facilities, providing leadership and oversight for the care delivered by dozens of physicians. With a deep dedication to the unique needs of older adults, Travis not only delivers expert primary care but also knows when to consult or refer within his trusted network of geriatric specialists to ensure the highest standard of care.
